DOH Conducting Maui Needs Assessment Survey

The Hawaiʻi Department of Health (DOH) is conducting a needs assessment survey for residents affected by the wildfires.

The purpose of the voluntary survey, which is currently being conducted via phone through October 11, is to assess ongoing priorities identified by the community. Collected information will help direct the continued work of DOH and other emergency response partners.

Selected households will be asked about their resource needs, access to medical and behavioral health support services, current physical health and well-being, and any other issues they have experienced in applying for and receiving necessary services.

All survey responses will be confidential, and no personally identifiable information will be collected.
